Acknowledgment and Assumption of Risk:

I, the undersigned, acknowledge that participation in the Mood Fashion Studio courses, workshops, and related activities ("Activities") involves certain risks, including but not limited to:

• Physical injuries from the use of sewing machines, cutting tools, fabrics, and other equipment.

• Damage or loss of personal property.

 

I understand that these risks may result in personal injury, illness, or property damage and willingly assume all such risks, whether known or unknown.

 

Release and Waiver of Liability:

In consideration of being permitted to participate in the Activities, I hereby release, waive, discharge, and covenant not to sue Mood Fashion Studio, its directors, officers, employees, volunteers, and agents ("Releasees") from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in the Activities or in any way related thereto.

 

Indemnification:

I agree to indemnify, defend, and hold harmless the Releasees from and against any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in the Activities.

 

Medical Treatment:

I grant permission to the Releasees to authorize emergency medical treatment as deemed necessary by the Releasees if I am injured or become ill while participating in the Activities. I agree to be responsible for any medical expenses incurred.

 

Photo/Video Release:

I grant Mood Fashion Studio permission to use photographs and/or video recordings of me taken during the Activities for promotional purposes, including but not limited to, social media, website, and marketing materials.

 

Severability:

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions will continue to be valid and enforceable to the fullest extent permitted by law.

 

Acknowledgment of Understanding:

I have read this Liability Waiver and Release Form, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law in New Jersey.
